Yolanda Tan Jacobs, age 61 of Clarksville passed away Saturday, November 8, 2014 at Gateway Medical Center. A Graveside Service will be held at 1:00 P.M., Thursday, November 13, 2014 at Elmwood Cemetery in Springfield, TN. Visitation will be 4:00 p.m. to 8:00 p.m. on Wednesday, November 12, 2014 and 11:00 a.m. to 12:00 p.m. on Thursday, November 13, 2014 at the funeral home. Burial will be in Elmwood Cemetery in Springfield, TN.
She was born October 9, 1953 in the Philippines. She was preceded in death by her husband, Harold Lee Jacobs, Sr. She was a housewife and dearly loved her grandchildren.
She is survived by her sons, Harold L. "Buddy" (Nancy) Jacobs,Jr., Alex (Christina) Jacobs, Mike (Monica) Jacobs; daughter, Kimberly Jacobs and 9 grandchildren.
Pallbearers will be Mike Jacobs, Robbie Roberson, Alex Jacobs, Dawson Jacobs, Zach Jacobs and Buddy Jacobs.
In lieu of flowers, contributions may be made to the American Heart Association, 1818 Patterson St., Nashville, TN 27203.
