Mary Jane Athey Polasek, 57, Clarksville, died Thursday, December 4, 2008, at Gateway Medical Center.
A memorial service will be held at 2:00 p.m. Sunday at Sykes Funeral Home Chapel with Father David Kennedy officiating.
Sykes Funeral Home & Crematory is in charge of arrangements.
Ms. Polasek was born September 12, 1951 in Belton, TX, daughter of the late Frank Manuel Toledo, Sr., and Bettie Jo Toledo. She was preceded in death by a brother Frank Manuel Toledo, Jr.
She was a retired Hair Stylist.
Ms. Polasek is survived by two children, Frank Michael Toledo, and Lisa Marie Cook. Also, survived by sisters Frances Felts, Vanleer; Joella Liggins, Clarksville; Michele Benge, Fayetteville, NC. and Faustina Toledo Mass, and two grandchildren.
